Your new account will have your historic health information transferred automatically.

SPH is transitioning to MyCareCorner portal. If you already have an account with the old platform, the next time you log in you will be asked to set up a new user name and password in our new patient portal platform, and you will be guided through the setup process for MyCare Corner. This platform is more user friendly and has improved features for your convenience.
