Referral or Appointment Request
You may request an appointment with a visiting specialist or your primary care provider may send a referral. Our team will help coordinate scheduling and confirm the date, time, and location of your visit.
Before Your Visit
You may be asked to complete intake forms and provide relevant medical history. Our staff will review available records in advance to ensure the specialist has the information needed for a focused, productive appointment.
Your Specialty Visit
During your visit, you will meet with the visiting specialist for a thorough evaluation. This may include discussion of your symptoms, review of prior testing, and, when appropriate, diagnostic evaluation.
Please bring your insurance information, a current medication list, and any relevant medical records that may not already be on file. Having this information helps us provide the most efficient and informed care possible.
Care Plan & Next Steps
Following your appointment, the specialist will outline recommendations. This may include additional testing, medication adjustments, in-office treatment, or follow-up planning. If further care is needed, our team will help coordinate the next steps.
Ongoing Coordination
After your visit, results and recommendations are shared with you and your referring provider to support clear communication and continuity of care within the community.
