Visiting the Family Care Clinic is designed to be straightforward, supportive, and focused on your overall health. While each visit is personalized depending on your needs, most appointments follow a similar process.
Before Your Appointment
You may receive appointment reminders by phone, text, or email. New patients may be asked to complete health history and consent forms in advance.
What to bring:
- Photo ID
- Insurance card
- Medication list (including over-the-counter medications and supplements)
- Relevant medical records, if available
Arrival and Check-In
When you arrive, front desk staff will confirm your information and insurance. Patients are typically asked to arrive about 15 minutes early, especially for first visits. You can complete any remaining forms or updates during this time.
Rooming and Assessment
A nurse or medical assistant will escort you to an exam room, review your reason for visit, update your health history, check vital signs, and review medications and allergies. For children, age-appropriate growth measurements and screenings may be included.
Time With Your Provider
Your provider will discuss your concerns, symptoms, or health goals, review your medical history and preventive care needs, perform an exam if needed, answer questions and explain findings, and recommend next steps. You’re encouraged to ask questions and share any concerns.
Tests, Procedures, or Vaccinations
Depending on your visit, lab work, vaccinations, minor procedures, or referrals may be ordered or completed.
After Your Visit
Before you leave, your care team will review your plan of care, medications, follow-up instructions, and next steps. Test results are typically shared through the patient portal or by phone. Medication refill requests can be made through the clinic, your pharmacy, or the portal. You are always welcome to call the clinic with any questions or concerns — we’re here to help!
