Medical Records and Health Information

South Peninsula Hospital utilizes Epic electronic health record for your medical records and care documentation, and MyChart is the patient portal for your access to your health information. However, sometimes we know you need to request copies of your medical records. We are happy to accommodate your needs, please utilize the forms at right in Quick Links.  If you have any questions regarding your request, please contact our Release of Information Office.

SPH participates in the exchange of health information for purposes of improved treatment and coordination of your care, for billing purposes and as required by law. These are secure statewide and/or nationwide exchanges. If you would like to limit the sharing of your information, please contact the hospital’s front desk.

Data Breach – SPH was informed of a potential patient information data breach in May, 2022 involving Professional Finance Company, Inc (PFC).  PFC notified all 250 patients with a mailed notice. Here is the letter (PDF) and official notice (PDF).
